| 척추경 나사못 고정술은 퇴행성 요추 질환의 치료에 사용되는 대표적인 수술적 치료 방법 중 하나이다. 본 연구에서는 골다공증 및 디스크 퇴행을 동반한 요추부(L1-S1)에서의 척추경 나사못 고정술의 생체역학적 영향을 평가하기 위하여 유한요소 연구를 수행하였다. 디스크 퇴행 및 골다공증에 따른 골질은 굴곡, 신전, 회전, 측굴곡 운동에 대한 요추부의 강성도와 회전 중심을 변화시켰다. 요추부의 골다공증을 구현하였을 때, 정상 요추 모델에 비하여 강성도는 최대 17.7%까지 감소하였으며, 디스크의 퇴행 정도가 심해질수록 더 감소하였다. 척추경 나사못의 시술은 시술 분절의 운동성을 감소시켰다. 운동 하중 조건에 따라 척추경 나사못을 삽입한 정상 요추는 인접 분절에서의 디스크 압력을 가장 크게 증가시켰으며, 척추경 나사못 시술 골다공증요추 모델, 정상 모델 그리고 골다공증 요추 모델 순으로 그 뒤를 따랐다. 척추경 나사못의 시술은 요추부의 골밀도 및 디스크의 퇴행에 따른 시술 분절에 미치는 영향을 줄이고 안정성을 제공하였으나, 요추부의 회전중심을 변화시킴으로 인접 분절의 운동성 및 디스크 내압을 증가시켜 추가적인 퇴행 가능성을 야기 시켰다. |

| Pedicle screw fixation(PSF) is widely used as the mainstay surgical treatment for degenerative lumbar disease. The purpose of this study was to assess biomechanical efficacies of the PSF in a lumbar spine(L1-S1) with osteoporosis and disc degeneration using finite element methods. Disc degeneration and bone quality due to osteoporosis altered the stiffness and center of rotation(COR) of a lumbar spine in flexion, extension, axial rotation and lateral bending. When osteoporosis was simulated, the stiffness was substantially decreased(by 17.7%). Compared to the normal disc, the stiffness was reduced along with degrees of disc degeneration. Implantation of PSF decreased range of motion(ROM) at the operative level. Normal spine plus PSF model showed the greatest increase in intradiscal pressures at the adjacent level, followed osteoporotic spine plus PSF, normal and osteoporotic model depending on the loading type. The treatment of PSF reduced the influence of the bone quality and disc degeneration at the operative level and provided stability. However, it changed COR in a lumbar spine, increased mobility and intradiscal pressures of the adjacent segment, resulting in additional adjacent segment disease. |
