Objective: Present an unusual case Target: Beginning practicionersMethods: To report a successful laparoscopic excision and repair of cesarean scar pregnancy in women with uterine didelphys with double vagina.Results: A 34-year-old woman with a history of previous one cesarean section was visited to our hospital due to suspect of having a cesarean scar pregnancy. She already confirmed that she had a uterine didelphys with double vagina during the infertility examination at 7 years ago. Vaginal ultrasound scan showed a 2cm sized gestational sac was checked at anterior wall of right uterus. Magnetic resonance imaging showed a 2.5cm sized gestational sac like cystic lesion in the lower segment of right uterus at cesarean scar. We decided to perform laparoscopic approach after consulting the patient with the operation method. First, both uterine arteries were ligated by metal clip to decrease the bleeding risk. The lower segment of previous cesarean site was excised with unipolar, minimizing bleeding. We could identify the gestational sac in lower segment of right uterus and that was evacuated by spoon forceps. The myometrium and serosa of anterior wall were each sutured in layer by a synthetic absorbable suture. No residual scar defect was visible on follow-up ultrasonography 1 month after surgery.Conclusion: Laparoscopic approach of a cesarean scar pregnancy in woman with uterine didelphys are effective and safe modes of treatment. In women with uterine anomalies, it is important to confirm the exact location of the gestational sac by preoperative imaging for successful surgery.
