큰 주제로 유방암에 대한 시나리오를 만들어 보기로 하고 Limit를 human, female, English로 하여 검색했고 그 결과 133,194건의 논문이 나와 검색어에 postmenopausal을 추가하여 6,312건으로 좁혔다. 여기에 다시 estrogen receptor positive를 검색어에 2차 추가하여 1,033건의 논문으로 제한하였다. 그 후 node-negative, tamoxifen, aromatase inhibitor 검색어를 더 넣어 최종적으로 11건의 논문을 검토하기로 하였다. 그렇게 선택된 11건의 논문은 1) 어떤 특정한 민족을 대상으로 한 연구, 2) 실험적인 증거 없이 이론상 기전으로만 설명한 논문, 3) 표본 집단이 너무 작은 경우를 제외하고 나니 5건의 논문이 남았다. 이렇게 얻은 4건의 논문을 좀 더 세밀하게 분석해 보았다. 1) Design, conduct, and analyses of Breast International Groups (BIG 1-98)의 경우에 Tamoxifen에서 letrozole로 대처하는 실험이 unblind이기에 그것을 이 실험으로 주로 하는 이 논문을 제외하였고, 2) The breast의 경우 기존 실험 결과에 대한 review만으로 작성된 논문이었기 에 제외하였다. 3) AI, efficacy and metabolic risk in the treatment of postmenopausal women with early breast cancer, 4) The use of 3rd-generation AI and tamoxifen in the adjuvant treatment of postmenopausal patients with hormone-dependent breast cancer의 경우도 review로만 이루어진 논문이었기에 금번 2-A의 가이드라인에서 제시하는 분석을 하기에 타당하지 않다는 판단으로 제외하였다.
☞ 최종 검색된 11개의 문헌들을 살펴본 결과 내 환자 case에 가장 적합하고 문헌의 신뢰성이 가장 높은 문헌을 선택할 수 있었다.
