본문내용

비삼출성 황반변성의 안저이상에 맥락막 혈관신생이나 원반 반흔이 동반되면 삼출성 노인성 황반변성으로 분류함. 노인성 황반변성 중 10%.

✓특징: 맥락막 혈관신생, 망막색소상피박리, 망막박리, 망막색소상피 파열 등의 증상 포함되면 결국 혈관성 섬유 반흔을 거쳐 원반 반흔이 초래.

✓역학, 임상 양상: 나이관련황반변성으로 인한 실명의 대부분(70~90%)이 삼출성 병변에 의한 것임

황반변성(비삼출성, 삼출성, 혼합형)과 BMI와의 연관성분석

1)적어도 7년이상 followed 된 것, 2)ARM과 유의하게 관련있는 것, 3)BMI와 흡연의 정보가 있는 것으로 선택
->256건의 비삼출성 ARM, 84건의 삼출성 ARM, 340건의 혼합 ARM

비삼출성 ARM : 정상 BMI에서 발생률이 최저. 정상 BMI와 비교했을 때
RR (CI) 값: 마름-1.43 과체중-1.24 비만- 2.15
삼출성 ARM : BMI와 유의한 결과가 없었고
혼합형 ARM : 비삼출성 타입과 비슷한 결과

비삼출성인 황반변성은 치료가 필요하지 않으나, 삼출성은 시력보존을 위한 적극적인 치료가 필요함.

변성이 일어난 부위의 경계를 명확히 알 수 있는 경우는 레이저광응고술을 시행하며, 광역학요법,항체주사, 초자체(유리체)절제술 등을 시행

아직까지 완전한 치료법은 없음.

완전한 치료를 위한 활발한 연구가 진행 중.
