The drug repayment price in health insurance is determined by the Japanese ministry of labor welfare.
The labor welfare ministry periodically investigates market price of pro drug products in order to include the actual-trading price into the drug pricing method.
The insurance drug pricing system of Japan is tightly woven to marketing structure of the pharmaceutical industry.
(1)-2 The changes of the drug pricing system
In 1958 the minister of labor welfare has established the current drug pricing system which determines the purchase price and is updated every 2 years.
The introduction of the Bulk Line method: by using the drug price investigation results as a reference, we line up the lowest trading costs of individual drug products and match 90% of the price as criteria of newly developed drugs. However such method can be easily manipulated by the seller’s strategy also the gap of similar product prices may derive largely.
The introduction of R-Zone method: if the weighted average of actual trade prices fall into a certain range in the drug pricing, we maintain the price and only add additional prices in case the price exceeds the range. This concept is based on the fact that there will be a rational range that such trading costs differences from the original price may be socially acceptable.
In 1991 the Central society medical insurance council suggested in changing the listed drug pricing method from ‘bulk-line’ to the ‘weighted average range’ method and so it was adapted from 1992 in the revision of drug pricing. The price range began with15% in 1992 and steadily declined to 5% in 1998. The discount rates are visible through the following graph.
